New Patient Forms

Welcome to Byron Center Family Dental!

For most adult patients, the first appointment will only include an exam with Doctor, perio charting, and x-rays. This appointment is to collect diagnostic information and establish you as a patient. Based on the health and history of your teeth and gums, we will schedule a return appointment for the appropriate dental cleaning. If you have had recent X-rays taken by your previous dental provider, please call them to request a transfer of records. You can have them emailed to byroncenterfamilydental@sb-dental.com.
